Family Diagnostic Questionnaire
Complete the questionnaire and email as an attachment to:

1.   How members do you have in your extended family?  Give a rough estimate.
2.   How many members of your extended family are under 20 years of age?
      a. How many members are 20 – 50 years of age?
      b. How many are 51 and over?  
3.   Where are your maternal and paternal families from, originally? County? City? State?
4.   Who are your oldest known paternal and maternal ancestors?
5.   Has your family collected historical documents relating to the family such as photos, newspaper clippings, wills, deeds,
      awards, and similar items that you consider the family archives?
6.   Where does most your extended family live today?
7.   How often do you have family reunions?
8.   Is there an expectation that family reunions will occur on a regular basis?
9.   Is there a committee consisting of different generations and genders that hail from different geographical areas that are
      involved in the planning of your family reunions?
10. Does your family have a historian who is a custodian for the family archives? 
11. Does your family have a resource database with contact information and resumes on all  
      of the adult members?
12. How do you currently keep track of your family members?
13. Is there a family member who regularly keeps in contact with other family members?
14. Is your average family per capita income over forty thousand dollars a year?
15. Does your family have a biography that tells the family story?
16. Does your family have an individual or group who has been formally identified as its leader?
17. Has your family incorporated as a non-profit organization?
18. Does your extended family have a family trust fund that is used for building wealth?
19. What percentage of the family has graduated from college?
20. Does your family have a charter with a mission and vision statement?
21. Does your family have its own traditions that all its members honor?
22. Is there a family member who leads the family in worship and praise? 
23. Does your family acknowledge the existence of God at family events? 
24. What percentage of your family is involved with organized religion?
25. Does your family have someone it identifies as the spiritual leader?    
26. Does your family have a slogan or symbol to identify it?
27. Are their stated and unstated expectations that act as a guide for family behavior? 
28. Have you identified any major challenges your family faces as a collective?
29. Do you have people in your family who are responsible for coordinating family events?  
30. Is there an electoral process that allows the most qualified individuals to be selected as leaders of the family?
31. Has your extended family compiled a health history?
32. Does your extended family have any type of preventive health program in place?
33. What gives you a greater sense of identity: race or family? 
34. Does your family have an identity separate from race?
35. If race were not an option how would you identify yourself?
36. Did your parents plan your conception?
37. What percentage of your family population was conceived according to a plan?
38. What percentage of your family population was born out of wedlock?
39. What is your family’s position on sex before marriage?
40. Is there a family member who takes charge when the family has an emergency?
41. Does your family have its own website or E-group?
42. What vehicle does your family use to resolve internal conflict?
43. What is the most common occupation found in your family?
44. What percentage of your family is unemployed?
45. What percentage of your family owns a business? 
46. Please describe what you think might improve your overall family experience.


After analyzing this data, Save the Family Institute will prepare a profile of your family.  If the family desires, the profile will be used to develop a Strategic Plan that is custom designed to maximize the family's operating proficiency.
